As a grantee of The Gus Schumacher Nutrition Incentive Program – Produce Prescription GusNIP, Family Health Centers of San Diego and UC San Diego Health are collaborating on the Produce Prescription Food as Medicine (PDP-FaM) program. The goal is to improve healthcare practices for patients with hypertension and/or type 2 diabetes in low-income, food-insecure communities across San Diego County by increasing access to and consumption of fresh fruits and vegetables (FF/V). The program utilizes a multicomponent approach, including a 12-month FF/V delivery and health education nutrition classes for participants to improve their knowledge of nutrition management. In addition, the program aims to demonstrate and evaluate the impact of FF/V prescriptions on increasing procurement and consumption of fruits and vegetables, reducing individual and household food insecurity, and reducing healthcare utilization and associated costs.
