FHCSD collaborated with the American Heart Association and the American Diabetes Association to provide education to patients with type 2 diabetes and their families. The primary goal is to help patients improve their knowledge and confidence about the link between type 2 diabetes, heart disease, and stroke, and how to take an active role in their healthcare. FHCSD Patient Care Support Services Department is comprised of registered dietitians, health educators, and nutritionists who, through individual counseling sessions, deploy KDBH resources and provide patients with diabetes care kits. In addition, this project aims to share the latest clinical guidelines to help healthcare providers and engage health systems in enhancing the quality of care for diabetics.
