Family Health Centers of San Diego was awarded the Relink Initiative Grant from the Center for Disease Analysis Foundation (CDAF). This effort is funded by Gilead Sciences, and the mission of the initiative is to support the development and implementation of programs that re-engage diagnosed-but-untreated hepatitis B- and C-infected individuals back into the care cascade across the United States. As a result of the Relink award, FHCSD aims to create a database to identify and track HCV/HBV diagnosed individuals; outreach to eligible HCV/HBV patients by bilingual Patient Navigators; schedule and track relink appointments; and contribute toward a joint manuscript with other Relink grantees.
