Family Health Centers of San Diego collaborated with the Moores Cancer Center (MCC) at University of California, San Diego Health to increase equitable access to colorectal cancer (CRC) screening and referral for care and treatment. Together, FHCSD and MCC will enhance patient education, case coordination, outreach, and other enabling services provided to patients. This study implemented a system-level intervention that includes an exchange of best practices, the deployment of MCC’s Outreach Specialist/Patient Navigator, workforce development and training, shared data/cancer statistics, the development of culturally competent and linguistically appropriate educational materials, as well as expansion of an electronic HIPAA-compliant system for bi-directional referrals.
